Job Description

Provide skilled nursing care to patients in their homes as a Home Health Registered Nurse (RN), delivering patient-centered services that promote health, independence, and comfort. This role requires independent clinical judgment, strong organizational skills, and effective communication with patients, families, and interdisciplinary teams.

Schedule/Hours:

  • Full-time, Monday through Friday
  • 8:00 AM to 5:00 PM

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify changes in condition, and coordinate with physicians and care teams.
  • Educate patients and families about disease management, medications, and preventive care.
  • Maintain accurate and timely documentation using Home Care Home Base (HCHB) electronic system.
  • Collaborate closely with therapists, social workers, and home health aides to ensure coordinated care delivery.
  • Participate in case conferences and adhere to all regulatory standards.

Qualifications:

  • Active RN license in good standing within the state of practice.
  • Minimum one year recent home health experience, preferably field-based.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
  • Strong clinical assessment, critical thinking, and communication skills.
  • Reliable transportation with valid drivers license and auto insurance.
  • Ability to work independently in a flexible, field-based schedule.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) preferred.
  • Experience managing 2532 points caseload weekly.
  • Knowledge of Medicare and home health compliance standards.
