Experienced and compassionate Home Health Registered Nurses (RNs) are needed to provide skilled nursing care directly to patients in their homes. This contract opportunity requires a self-motivated, organized clinician who thrives working independently while delivering patient-centered care and maintaining exceptional clinical standards.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Field-based: daily travel within a designated territory (mileage reimbursement provided)

Key Responsibilities:

• Conduct comprehensive patient assessments using OASIS; develop and manage individualized care plans.
• Deliver skilled nursing including medication administration, wound care, IV therapy, and chronic disease management.
• Monitor patient progress, promptly address changes in condition, and collaborate with the interdisciplinary care team and physicians.
• Educate patients and families on care plans, medication, and health management.
• Document care accurately and promptly using Home Care Home Base (HCHB).
• Participate in team conferences, ongoing education, and ensure all care meets current regulatory standards.

Qualifications:

• Active RN license in good standing (state-specific)
• Minimum 1 year of recent home health nursing experience (field experience preferred)
• Proficiency with HCHB required; OASIS experience required
• Strong clinical assessment, time management, and organizational skills
• Reliable vehicle, valid drivers license, and current auto insurance
• Ability to work independently and adapt to a field-based schedule

Preferred:

• BSN degree
• Experience with a 2532-point caseload per week
• Knowledge of Medicare and home health compliance standards

$1,845 - $1,952/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
