An exciting opportunity is available for a dedicated and skilled Registered Nurse with a passion for home health care. In this field-based role, you'll deliver high-quality, patient-centered nursing services directly to patients in their residences, ensuring continuity of care while fostering patient independence and well-being. This is a full-time, 13-week contractideal for professionals seeking a dynamic, autonomous environment that allows meaningful impact on patient outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active RN license (in good standing) in the state of practice
- At least 1 year of recent Home Health nursing experience (field-based preferred)
- Strong competency in Home Care Home Base (HCHB)
- OASIS assessment experience required
- Excellent clinical assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Self-motivated, able to effectively manage a flexible, independent schedule
Preferred:
- BSN degree
- Demonstrated experience with 2532 point weekly caseloads
- Knowledge of Medicare and home health compliance
Core Responsibilities:
- Complete OASIS assessments and create individualized care plans
- Provide skilled nursing interventions (wound care, medication administration, IV therapy, and chronic disease management)
- Monitor patient progress, recognize changes in condition, and collaborate with interdisciplinary teams
- Educate patients and caregivers on disease management and preventive care
- Ensure timely and accurate documentation in HCHB
- Attend case conferences, staff meetings, and pursue ongoing professional development
- Maintain adherence to regulatory and agency clinical standards
