Seeking a skilled Home Health Registered Nurse (RN) to deliver high-quality patient care in a field-based role. Provide personalized nursing support to patients in their homes, ensuring comprehensive and compassionate care while working autonomously. As an integral part of the home health team, you'll perform OASIS assessments, manage care plans, and collaborate with interdisciplinary professionals to achieve positive patient outcomes.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Typical caseload is 2532 points weekly (approximately 35 patient visits per day)
- Travel required within assigned territory; mileage reimbursement provided
Key Responsibilities:
- Complete OASIS assessments to evaluate patient needs and progress
- Develop, implement, and manage individualized care plans
- Deliver skilled nursing care, such as wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress and communicate with physicians and care teams
- Provide patient and family education on disease management, treatment plans, and preventive care
- Accurately document care using Home Care Home Base (HCHB)
- Collaborate with therapists, social workers, and home health aides to ensure coordinated care
- Participate in staff meetings, case conferences, and continued education
Qualifications:
- Current, active RN license in state of practice (in good standing)
- At least 1 year of recent home health (field) experience
- Strong proficiency in OASIS and Home Care Home Base (HCHB)
- Outstanding assessment and clinical skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to manage a flexible, independent schedule
- Excellent communication and organizational skills
- BSN preferred
- Knowledge of Medicare/home health compliance standards a plus
