Experienced registered nurses passionate about patient-centered, in-home care will thrive in this contract opportunity. This role focuses on delivering high-quality clinical support to individuals in their homes, empowering them on their path to better health while fostering positive outcomes through strong professional partnerships.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Daily travel within assigned territory (mileage reimbursement provided)
- Caseload: 2532 points per week, typically 35 patient visits per day
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
- Track progress, identify and report changes in condition, and communicate efficiently with interdisciplinary teams
- Educate patients and families on disease management, medications, and preventive health
- Document promptly and accurately using Home Care Home Base (HCHB)
- Participate in case conferences, staff meetings, and ongoing professional development
- Support regulatory compliance and high standards for patient care delivery
Minimum Qualifications:
- Active, unencumbered RN license in the state
- At least 1 year of recent home health experience, with field care preferred
- Proficient in Home Care Home Base (HCHB) and OASIS
- Reliable transportation, valid drivers license, and up-to-date auto insurance
- Exceptional clinical judgment, communication, and time management skills
Preferred Qualifications:
- BSN degree
- Experience managing 2532 points per week
- Familiarity with Medicare and home health regulations
