Deliver high-quality, patient-centered nursing care directly in the comfort of patients homes with this engaging field-based opportunity as a Home Health Registered Nurse (RN). This full-time, 13-week contract role focuses on skilled nursing, individualized care planning, and interdisciplinary teamwork to achieve optimal outcomes for home health patients.

Schedule & Structure:

• Monday to Friday, 8:00 AM 5:00 PM
• Caseload expectation: 2532 points per week (typically 35 patient visits each day)
• Daily travel within a defined territory (mileage reimbursement provided)
• Electronic documentation via Home Care Home Base (HCHB) and OASIS assessments

Key Responsibilities:

• Conduct comprehensive patient assessments and create individualized, OASIS-driven care plans.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor clinical status, track patient progress, and update care team and providers on significant changes.
• Educate patients and families about diagnoses, treatments, medication, and preventive strategies.
• Maintain accurate, prompt clinical documentation in HCHB.
• Collaborate with an interdisciplinary team of therapists, social workers, and aides to ensure coordinated, holistic care.
• Attend case conferences, staff meetings, and ongoing training as required.
• Uphold all regulatory and professional standards for documentation and care delivery.

Required Qualifications:

• Active Registered Nurse (RN) license in good standing
• Minimum 1 year of recent home health experience (field-based strongly preferred)
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments
• Excellent communication, time management, and organizational skills
• Reliable transportation, valid drivers license, and active auto insurance

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN)
• Demonstrated experience managing a 2532 point weekly caseload
• Strong understanding of Medicare & home health regulations
