Deliver exceptional patient-centered care as a Home Health Registered Nurse. This role is pivotal in supporting individuals recovering or managing chronic illnesses within the comfort of their homes. It is ideal for experienced nurses who excel at working independently, thrive in the field, and are dedicated to clinical excellence.

Position Details:

• Schedule: Monday Friday, 8:00 AM 5:00 PM
• Duration: 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Setting: Providing care throughout the community in patient homes
• Documentation: Home Care Home Base (HCHB)
• Assessment: OASIS tool utilized
• Mileage reimbursement provided

Primary Responsibilities:

• Conduct thorough OASIS assessments and develop tailored care plans
• Deliver hands-on skilled nursing care, including medication management, wound and IV care, and chronic disease monitoring
• Track patient progress, identify new needs or risks, and communicate effectively with physicians and interdisciplinary teams
• Educate patients and families on disease management, medications, and overall health maintenance
• Ensure accurate and timely documentation using HCHB
• Collaborate with colleagues such as therapists, social workers, and aides to support positive health outcomes
• Participate in ongoing training, meetings, and case conferences to maintain compliance and best practices

Qualifications:

• Active, unrestricted RN license
• At least 1 year of recent home health experience (field-based strongly preferred)
• Proficiency with HCHB and OASIS assessments required
• Strong assessment, clinical judgment, and independent time management skills
• Reliable transportation with valid drivers license and auto insurance
• BSN degree, prior management of a 2532 point caseload, and Medicare knowledge are preferred

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
