An opportunity awaits for an experienced and compassionate Registered Nurse ready to excel in the field of home health. This full-time, 13-week contract role combines both clinical skill and the rewarding experience of delivering direct care to patients in the comfort of their homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

What You'll Do:

• Manage a caseload averaging 2532 points weekly, with approximately 35 patient visits per day.
• Travel daily to patient residences within your assigned territory (mileage reimbursement provided).
• Perform OASIS assessments and develop personalized care plans.
• Deliver a range of nursing care: wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify status changes, and coordinate timely communication with the clinical team and physicians.
• Educate patients and families on health conditions, treatment protocols, medication regimens, and preventive actions.
• Ensure accurate, prompt documentation using Home Care Home Base (HCHB).
• Collaborate with an interdisciplinary teamtherapists, social workers, and home health aidesto ensure cohesive patient care.
• Participate in care conferences, staff meetings, and ongoing professional development.

Qualifications:

• Active RN license in good standing within the state.
• At least 1 year of recent home health nursing experience (field experience strongly preferred).
• Demonstrated proficiency in HCHB and OASIS assessments.
• Strong assessment capabilities and clinical decision-making skills.
• Ability to work independently with excellent organizational and communication skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• BSN and Medicare home health compliance knowledge are a plus.
