Job Description
Deliver skilled, patient-centered nursing care in a field-based Home Health RN position supporting individuals in their own homes. This contract opportunity offers a dynamic environment where youll work independently, manage your caseload, and play a key role in promoting positive outcomes for patients across Germantown, TN.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Expected caseload: 2532 points per week (typically 35 patient visits per day)
- Daily travel required within territory (mileage reimbursement provided)
Qualifications:
- Current and active RN license (in good standing)
- At least 1 year of recent home health experience (field work strongly preferred)
- Solid proficiency in Home Care Home Base (HCHB)
- OASIS assessment experience required
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Outstanding time management, communication, and organizational abilities
- BSN preferred and knowledge of Medicare/home health compliance standards a plus
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Provide skilled care including wound care, IV therapies, medication management, and disease education
- Monitor patient progress, document thoroughly in HCHB, and communicate with care teams
- Educate patients and families regarding diagnosis, medication, and preventive care
- Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides
- Uphold regulatory and agency standards in delivering care and documentation
- Attend staff meetings, case conferences, and ongoing training as needed
