This full-time, 13-week contract role offers a rewarding mix of patient interaction and collaboration with interdisciplinary healthcare partners.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Field-based with daily independent travel to patient homes
- Caseload: 2532 points per week (approximately 35 visits per day)
Required Qualifications:
- Active RN license (in good standing)
- Minimum 1 year of recent home health nursing experience
- Proficiency in Home Care Home Base (HCHB) electronic documentation
- Prior OASIS assessment experience
- Strong evaluation, critical thinking, and organizational skills
- Reliable transportation with valid drivers license and insurance
- Ability to communicate clearly and prioritize effectively
Preferred Experience:
- BSN
- Experience managing a home health caseload of 2532 points per week
- Familiarity with Medicare and compliance standards
Primary Responsibilities:
- Conduct OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions: wound care, IV therapy, medication administration, chronic disease management
- Educate patients and caregivers on treatment plans, medications, and preventive care
- Document timely and accurately in HCHB
- Communicate changes in condition to physicians and the team
- Participate in collaborative conferences and staff meetings
