Embark on a rewarding career delivering essential in-home healthcare as a skilled Registered Nurse specializing in home health services. This contract position provides a dynamic environment where autonomy, empathy, and clinical expertise are key to helping patients achieve optimal outcomes while remaining in the comfort of their homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct thorough OASIS assessments and craft individualized care plans to address each patients unique needs.
- Deliver skilled nursing interventions, including wound care, IV therapy, medication management, and chronic illness support.
- Closely monitor patient conditions, promptly update care teams and physicians, and adjust care plans as necessary.
- Offer clear education to patients and families about diagnoses, treatment regimens, medication, and preventive care strategies.
- Accurately document assessments and interventions using the Home Care Home Base (HCHB) platform.
- Collaborate with allied health professionals like therapists, medical social workers, and home health aides to ensure holistic care.
- Contribute to case conferences, attend staff meetings, and pursue ongoing learning opportunities.
- Uphold rigorous standards for documentation, patient safety, and compliance with regulatory guidelines.
Qualifications:
- Active registered nursing license in good standing.
- At least one year of recent home health (field-based) nursing experience.
- Proficient with OASIS and Home Care Home Base (HCHB) documentation systems.
- Demonstrated clinical assessment skills, attention to detail, and strong critical thinking.
- Reliable transportation, valid drivers license, and active auto insurance.
- Excellent interpersonal, communication, and time-management abilities.
- BSN and experience with Medicare compliance preferred.
