Job Description

Step into a vital nursing role providing compassionate care to patients in their homes. This full-time, 13-week contract position is designed for Registered Nurses with a strong background in home health. Youll make a real difference by delivering personalized, patient-centered care and ensuring continuity throughout every stage of the patient journey.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and design individualized care plans.
• Deliver skilled nursing, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, quickly identify and report changes in condition, and coordinate with the care team.
• Provide clear education to patients and families on disease processes, medications, treatments, and preventive strategies.
• Maintain accurate documentation within the Home Care Home Base (HCHB) electronic system.
• Work collaboratively with physical and occupational therapists, social workers, and aides for integrated care delivery.
• Attend case conferences, meetings, and pursue ongoing educational opportunities.
• Uphold agency and regulatory standards in all aspects of care and documentation.

Qualifications:

• Active RN license in good standing in the state of practice.
• At least 1 year of recent home health nursing experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS.
• Strong clinical assessment and problem-solving abilities.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to work independently, organize a flexible field-based schedule, and excel in critical communication.

Preferred:

• BSN degree.
• Previous experience managing 2532 points per week.
• Familiarity with Medicare and home health compliance.
