Job Description

Expand your nursing career by caring for patients in their own homes as an experienced Home Health Registered Nurse. This contract position offers the opportunity to make a meaningful impact through direct patient care and education, while collaborating closely with a supportive interdisciplinary team.

Schedule:

  • MondayFriday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Complete comprehensive OASIS assessments and develop personalized care plans.
  • Deliver skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management.
  • Track and report patient progress, promptly identifying and communicating changes in condition.
  • Provide thorough education to patients and families about diseases, treatments, medications, and preventive care.
  • Accurately document care activities using Home Care Home Base (HCHB).
  • Coordinate with therapists, medical social workers, and home health aides for holistic, patient-centered care.
  • Attend case conferences and ongoing staff meetings for continuous professional development.
  • Ensure compliance with regulatory guidelines and agency standards in both documentation and care delivery.

Qualifications:

  • Active, unrestricted RN license in the state of practice.
  • Minimum of 1 year of recent field home health nursing experience.
  • Proficient with Home Care Home Base (HCHB) and OASIS assessment.
  • Excellence in clinical assessment, organization, and communication skills.
  • Comfortable working independently; able to effectively manage a flexible, field-based schedule.
  • Valid drivers license, reliable transportation, and auto insurance required.
  • BSN, previous experience managing 2532 points/week, and knowledge of Medicare/home health compliance are preferred.
