Job Description

Deliver comprehensive nursing care to patients in their homes by providing skilled, compassionate support as a Home Health Registered Nurse (RN). This role requires autonomy, strong clinical expertise, and excellent communication to ensure patient-centered care and positive health outcomes. You will manage caseloads efficiently, perform detailed assessments, and collaborate with interdisciplinary teams to maintain high standards of care.

Key Responsibilities:

  • Conduct thorough OASIS assessments and develop individualized care plans.
  • Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, recognize changes in condition, and communicate with physicians and care teams promptly.
  • Educate patients and families on health conditions, treatments, medications, and preventive care practices.
  • Maintain accurate, timely electronic documentation in Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and home health aides to ensure integrated care.
  • Participate in case conferences and ongoing professional education.
  • Adhere to all regulatory and agency standards for documentation and patient care delivery.

Qualifications and Desired Experience:

  • Current RN license in good standing.
  • Minimum of 1 year recent home health nursing experience; field experience preferred.
  • Proficient in Home Care Home Base (HCHB) and OASIS.
  • Strong clinical assessment and critical thinking skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently and manage a flexible, field-based schedule.
  • Excellent communication, time management, and organizational abilities.
  • BSN preferred.
  • Prior experience managing 2532 points per week and knowledge of Medicare and home health compliance standards is a plus.

Contract Details:

  • Full-time, 13-week contract
  • Monday through Friday, 8:00 AM 5:00 PM
  • Caseload: approximately 2532 points weekly (35 patient visits per day)
  • Daily travel to patient homes within assigned territory with mileage reimbursement
