An experienced and compassionate Registered Nurse is needed to deliver exceptional home health care to patients in a field-based setting. This contract opportunity prioritizes clinical excellence, patient-centered service, and independent work. The Home Health RN will manage patient visits, deliver skilled nursing care, and ensure ongoing communication with interdisciplinary teams to support patient recovery and wellness at home.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Travel required within the assigned territory (mileage reimbursement provided)

Responsibilities:

• Conduct comprehensive OASIS assessments and develop patient-focused care plans.
• Provide skilled nursing interventions, such as wound care, medication administration, chronic disease management, and IV therapy.
• Monitor and evaluate patient progress, identifying changes and collaborating with care teams and physicians as needed.
• Educate patients and caregivers about diagnoses, medications, and preventative health.
• Complete accurate, timely documentation using Home Care Home Base (HCHB).
• Coordinate with physical therapy, occupational therapy, social work, and home health aides for integrated care delivery.
• Participate in case conferences, professional development, and staff meetings.
• Maintain compliance with regulatory standards and agency policies.

Qualifications:

• Active and unrestricted RN license in the state of practice
• At least 1 year of recent home health nursing experience
• Proficiency using Home Care Home Base (HCHB) and with OASIS documentation
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment, communication, and time management skills
• Ability to work autonomously in a field-based role
• BSN and prior experience managing 2532 points per week preferred
• Familiarity with Medicare and home health regulations is a plus

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
