Job Description
Elevate patient care by making a difference as a Home Health Registered Nurse. This full-time, 13-week contract position offers the chance to deliver skilled nursing support directly to patients within their homes, creating a rewarding experience that prioritizes continuity, comfort, and patient-centered outcomes.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient conditions, reporting promptly to interdisciplinary healthcare teams and physicians.
- Educate patients and families on diagnoses, treatment plans, medications, and preventive care for optimal recovery.
- Maintain accurate, timely documentation within the Home Care Home Base (HCHB) system.
- Collaborate seamlessly with therapists, medical social workers, and home health aides to facilitate holistic, coordinated care.
- Attend case conferences, participate in staff meetings, and engage in continuing education.
- Uphold all regulatory guidelines and agency standards for patient care and documentation.
Required Qualifications:
- Active and unencumbered RN license in the relevant state (good standing).
- Minimum 1 year of recent, hands-on Home Health nursing experience (field-based care preferred).
- Demonstrated proficiency in HCHB and OASIS.
- Strong assessment, organizational, and communication abilities.
- Valid driver's license, reliable transportation, and active auto insurance.
- Ability to thrive independently while managing a dynamic field-based caseload.
Preferred Skills:
- Bachelor of Science in Nursing (BSN).
- Proven experience managing 2532 points per week.
- Familiarity with Medicare and current home health compliance standards.
