Bring your clinical expertise and compassion to a vital home health role, delivering patient-centered care directly in the community. This field-based Registered Nurse position offers a meaningful opportunity to support patients in their homes, fostering independence and improved outcomes through individualized care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: 2532 points per week (typically 35 patient visits daily)
Key Responsibilities:
- Conduct thorough OASIS assessments, creating and managing customized care plans.
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and document patient progress in Home Care Home Base (HCHB); promptly communicate changes with the care team and physicians.
- Educate patients and families about care plans, medications, and disease management.
- Maintain accurate, regulatory-compliant records.
- Collaborate with an interdisciplinary team, participate in case conferences and ongoing education.
- Travel within the assigned territory, with mileage reimbursement provided.
Qualifications:
- Active RN license in good standing for the state.
- At least 1 year of recent home health RN experience (field preferred).
- Proficient in Home Care Home Base (HCHB) and experienced in OASIS.
- Reliable transportation, valid drivers license, and current auto insurance.
- Skilled in time management, independent work, and patient communication.
- Preferred: BSN, familiarity with 2532 point caseload, knowledge of Medicare and home health regulations.
