Job Description

Make a meaningful impact in the lives of patients and families with compassionate hospice care. In this role, you will deliver skilled nursing services in home and residential settings while ensuring comfort and dignity for those facing life-limiting illnesses. You will work collaboratively to provide holistic support throughout every stage of the hospice journey.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation may be required

Key Responsibilities:

  • Conduct comprehensive nursing assessments and develop individualized care plans
  • Deliver symptom management, pain control, and focused comfort measures
  • Administer medications and treatments according to physician orders and hospice protocols
  • Monitor patient conditions, documenting and communicating changes via Home Care Home Base (HCHB)
  • Educate patients, families, and caregivers on managing symptoms, administering medication, and emotional support
  • Collaborate with a multidisciplinary team, including social workers, chaplains, aides, and volunteers
  • Ensure timely, accurate documentation and compliance with regulatory guidelines
  • Participate in case conferences, team meetings, and ongoing quality improvement projects

Qualifications & Experience:

  • Current RN license (in good standing)
  • Minimum of 1 year of recent hospice experience
  • Proficiency with the Home Care Home Base (HCHB) documentation system
  • Strong clinical, assessment, and communication skills
  • Understanding of pain management and family-centered support
  • Reliable transportation, valid drivers license, and auto insurance
  • Independent, organized, and compassionate in approach
  • BSN and experience with Medicare hospice requirements preferred
