Make an immediate impact in patient care as a dedicated Home Health Registered Nurse (RN) providing skilled nursing services in a rewarding, field-based role. This full-time, 13-week contract offers a MondayFriday, 8:00 AM 5:00 PM schedule, with a manageable caseload of 2532 points per week (typically 35 patient visits per day). Youll enjoy daily variety and autonomy as you travel to patients homes within your assigned territory, supported by mileage reimbursement and leading technology.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver high-quality, skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, identify condition changes, and communicate effectively with healthcare team members and physicians
• Provide patient and family education on treatment plans, medications, and preventive care
• Maintain accurate and timely documentation in Home Care Home Base (HCHB)
• Collaborate with therapy staff, social workers, and home health aides to ensure excellent, coordinated care
• Participate in case conferences, staff meetings, and ongoing professional education
• Adhere to regulatory and agency standards

Required Qualifications:

• Active, unencumbered RN license in the state of practice
• A minimum of one year of recent Home Health field experience
• Proficiency in Home Care Home Base (HCHB) and OASIS documentation
• Strong clinical assessment, communication, and organizational skills
• Reliable transportation, valid driver's license, and auto insurance
• Ability to work independently and manage a flexible, field-based schedule

Preferred Qualifications:

• BSN degree
• Experience managing a 2532 point weekly caseload
• Familiarity with Medicare and home health compliance

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
