Step into a vital role delivering skilled nursing care to patients in the comfort of their homes as a Home Health Registered Nurse (RN). This contract opportunity is ideal for a self-driven, compassionate RN eager to make a difference through high-quality, patient-centered care and collaborative, multidisciplinary teamwork.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Contract duration: 13 weeks
• Typical caseload: 2532 points per week (approximately 35 patient visits daily)
• Field-based, with daily travel to patient residences (mileage reimbursement provided)

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop personalized care plans.
• Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and communicate effectively with physicians and clinical teams.
• Provide thorough education to patients and families regarding conditions, treatments, medications, and preventive health.
• Ensure detailed, timely documentation in Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to ensure seamless, coordinated care.
• Participate in case conferences, staff meetings, and ongoing professional education.
• Comply with regulatory standards and internal protocols.

Qualifications:

• Active RN license (state of practice), in good standing.
• At least one year of recent Home Health RN experience (field experience preferred).
• Proficient in Home Care Home Base (HCHB) and OASIS.
• Outstanding assessment, communication, and organizational skills.
• Reliable transportation, valid drivers license, and appropriate insurance.
• BSN preferred; experience managing 2532 points per week and knowledge of Medicare/home health compliance standards are a plus.
