Job Description
Deliver exceptional hospice care and compassionate support as a Registered Nurse specializing in end-of-life services. In this contract opportunity, you'll provide comfort, dignity, and quality of life to patients and their families in home and residential settings. You'll also collaborate with a dedicated interdisciplinary team to coordinate care and facilitate a patient-centered experience during each phase of the hospice journey.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation as needed
- Expect approximately 45 patient visits daily
Key Responsibilities:
- Conduct comprehensive nursing assessments and develop individualized hospice care plans
- Deliver skilled nursing care focused on symptom management and patient comfort
- Administer medications and treatments per physician orders and established protocols
- Monitor patient status, reporting changes promptly to the hospice medical director and team
- Educate patients, families, and caregivers on hospice care, disease progression, medication administration, and comfort measures
- Partner with social workers, chaplains, aides, and volunteers to address holistic needs
- Complete accurate and timely documentation using Home Care Home Base (HCHB)
- Participate in care conferences, team meetings, and ongoing quality improvement activities
- Adhere to all regulatory and hospice-specific policies and guidelines
Qualifications:
- Active RN license in good standing
- Minimum one year of recent hospice nursing experience
- Proficiency with Home Care Home Base (HCHB)
- Excellent clinical assessment and communication skills
- Strong understanding of end-of-life care principles and pain management
- Demonstrated ability to work independently and manage a field-based schedule
- Reliable transportation and a valid drivers license
- Compassionate and emotionally resilient
Preferred:
- BSN
- Experience in palliative or end-of-life settings
- Familiarity with Medicare hospice regulations
