Job Description
Providing compassionate end-of-life care as a Hospice Registered Nurse (RN) involves delivering skilled nursing and emotional support to patients and families in home and residential settings. This full-time contract opportunity requires a dedicated RN to manage a caseload of approximately 45 patients daily throughout Fresno, CA. The role emphasizes patient comfort, symptom management, and interdisciplinary collaboration to ensure quality hospice care.
Responsibilities:
- Perform comprehensive nursing assessments and develop individualized hospice care plans
- Administer medications and treatments aligned with physician orders and hospice protocols
- Monitor and report changes in patient condition promptly to medical directors and care teams
- Educate patients and families about disease progression, symptom relief, and caregiving strategies
- Collaborate with social workers, chaplains, CNAs, and volunteers to address holistic patient needs
- Accurately document care and patient progress using Home Care Home Base (HCHB)
- Participate in care conferences, team meetings, and quality improvement initiatives
- Adhere to all hospice policies, procedures, and regulatory requirements
Qualifications:
- Active RN license in the state (good standing)
- Minimum 1 year recent hospice nursing experience
- Proficiency with Home Care Home Base (HCHB) documentation system
- Strong clinical assessment and communication skills
- Knowledge of pain management and end-of-life care principles
- Reliable transportation with valid drivers license and auto insurance
- Ability to work independently in a field-based setting
- Compassionate and resilient approach to patient care
Preferred:
- Bachelor of Science in Nursing (BSN)
- Experience in palliative care or end-of-life settings
- Familiarity with Medicare hospice regulations and documentation
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Occasional weekends and on-call rotation may be required
