An excellent opportunity awaits for a skilled and compassionate Registered Nurse with proven experience in home health care. This full-time, 13-week contract role focuses on delivering exceptional, patient-centered care directly in patients homes, supporting positive clinical outcomes and continuity of care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
As a Home Health RN, you will manage a caseload of 2532 points per week (typically 35 patient visits per day) within a designated territory, using the Home Care Home Base (HCHB) electronic documentation system and the OASIS assessment tool. Mileage reimbursement supports your daily travel to each patients home.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized plans of care
- Deliver skilled nursing services such as medication administration, wound care, IV therapy, and chronic disease management
- Monitor patient progress, recognize changes in condition, and quickly update the care team and physicians
- Educate patients and their families about health conditions, care plans, medications, and preventive care
- Maintain accurate and timely documentation in HCHB
- Collaborate with therapists, social workers, and home health aides to ensure coordinated, multidisciplinary care
- Attend case conferences and participate in professional development activities
- Adhere to all regulatory and agency standards for patient care and documentation
Required Qualifications:
- Active, unencumbered RN license for the state of practice
- Minimum 1 year recent home health nursing experience, preferably field-based
- Proficient in using HCHB and OASIS
- Strong assessment, clinical judgment, and organizational skills
- Reliable transportation, valid drivers license, and insurance
Preferred:
- BSN degree
- Experience managing 2532 points weekly
- Familiarity with Medicare and home health compliance

$1,770 - $1,873/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
