Deliver expert nursing care as a Home Health Registered Nurse (RN), providing compassionate, patient-focused services within the comfort of patients homes. This full-time, 13-week contract role requires a self-motivated RN to manage a caseload averaging 2532 points weekly, typically involving 35 daily patient visits throughout Franklin, KY, and surrounding areas. Emphasis is placed on clinical excellence, clear communication, and independent decision-making in a field-based setting.

Responsibilities:

• Perform comprehensive OASIS assessments and develop personalized care plans.
• Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, detecting changes, and coordinating promptly with physicians and the interdisciplinary care team.
• Educate patients and families on disease management, medications, and preventive care.
• Maintain accurate and timely electronic documentation using Home Care Home Base (HCHB).
• Collaborate effectively with therapists, social workers, and home health aides to ensure seamless, coordinated care.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Adhere to regulatory and agency standards ensuring quality and compliance.

Qualifications:

• Active RN license in good standing for the state.
• Minimum one year recent home health nursing experience (field experience preferred).
• Proficient with Home Care Home Base (HCHB) and OASIS documentation tools.
• Strong clinical assessment, critical thinking, communication, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible schedule.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 weekly points.
• Familiarity with Medicare and home health compliance standards.
