An opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health. This dynamic field-based role focuses on delivering expert care to patients in their homes, fostering independence while maintaining a high standard of nursing excellence. Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions, including wound management, medication administration, IV therapy, and chronic disease care.
• Evaluate and document patient progress using the Home Care Home Base (HCHB) electronic system.
• Educate patients and caregivers on treatment regimens, disease processes, medications, and preventive strategies.
• Work collaboratively with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated, quality care.
• Communicate changes in condition promptly to physicians and interdisciplinary teams.
• Participate in case conferences, staff meetings, and ongoing education.
• Maintain adherence to regulatory standards and agency requirements.

Qualifications:

• Active, unrestricted RN license in the state.
• At least 1 year of recent home health field experience.
• Proficiency with HCHB and OASIS is mandatory.
• Strong judgement, organizational, and documentation abilities.
• Valid drivers license, reliable vehicle, and auto insurance.
• Ability to independently manage schedules and multiple priorities.
• BSN and experience managing 2532 points weekly (preferred).
• Familiarity with Medicare and home health compliance standards.
