Job Description
An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patients homes within Floyd, VA. This contract position offers a full-time schedule from Monday through Friday, 8:00 AM to 5:00 PM, with a 13-week duration. The role requires managing a caseload averaging 2532 points weekly, typically consisting of 35 patient visits per day.
Responsibilities:
- Conduct thorough OASIS assessments and develop individualized care plans.
- Administer skilled nursing services including wound care, medication management, IV therapy, and chronic disease monitoring.
- Monitor patient progress, recognize condition changes, and communicate effectively with physicians and the care team.
- Educate patients and families on treatment plans, medications, and preventive care.
- Maintain accurate documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary team members to ensure coordinated patient care.
- Participate in case conferences, staff meetings, and continuing education.
- Comply with all regulatory and agency standards related to patient care and documentation.
Qualifications:
- Active and valid RN license in the state of practice.
- At least 1 year of recent home health experience with proven proficiency in HCHB and OASIS assessments.
- Strong clinical assessment and critical thinking abilities.
- Reliable transportation, valid drivers license, and auto insurance to support daily travel within assigned territories.
- Ability to work independently and manage a flexible, field-based schedule.
- Excellent communication, organization, and time-management skills.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience handling 2532 weekly points caseload.
- Knowledge of Medicare and home health compliance standards.
