An opportunity is available for a dedicated and knowledgeable Home Health RN to provide quality care to patients in their homes. This contract position offers a chance to make a real impact on the health and well-being of individuals, combining independent work with strong team collaboration.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and create/evaluate individualized care plans.
• Deliver skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease support.
• Monitor patient progress, recognize changes in condition, and communicate promptly with physicians and interdisciplinary team members.
• Educate patients and families about health conditions, medications, care plans, and preventive measures.
• Maintain accurate documentation in Home Care Home Base (HCHB) and ensure all regulatory standards are met.
• Coordinate care with other professionals, such as physical/occupational therapists, social workers, and home health aides.
• Participate in team meetings, case conferences, and ongoing professional development.

Qualifications:

• Active RN license (state of practice) in good standing.
• At least one year of recent home health experience (field experience preferred).
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment, organizational, and time management skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to operate independently in the field with excellent communication skills.
• Preferred: BSN and experience managing a caseload of 2532 points per week, including knowledge of Medicare compliance.
