Job Description
Deliver high-quality, patient-centered care as a Home Health Registered Nurse, providing vital support to patients in the comfort of their own homes. This full-time, 13-week contract is ideal for professionals who thrive with autonomy, possess excellent clinical and assessment skills, and have a strong commitment to both patient outcomes and regulatory compliance.
Schedule & Structure:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based: travel daily to patient homes within your assigned territory (mileage reimbursement included)
- Manage a caseload of 2532 points/week (35 patient visits per day)
- Use Home Care Home Base (HCHB) for documentation and perform OASIS assessments
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans using OASIS
- Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
- Identify and promptly report changes in patient condition to the interdisciplinary team and physicians
- Educate patients and family members on their care plan, treatment, and preventive strategies
- Ensure accurate, timely documentation in HCHB
- Work collaboratively with therapists, social workers, and home health aides
- Attend case conferences and participate in continuing education
Required Qualifications:
- Current RN license (in good standing) in the state of practice
- Minimum 1 year of recent home health experience
- Proficient in Home Care Home Base (HCHB) and OASIS
- Strong clinical judgment, organizational, and communication abilities
- Reliable transportation, valid drivers license, and auto insurance
Preferred Attributes:
- BSN degree
- Experience managing caseloads of 2532 points per week
- Knowledge of Medicare/home health compliance
