Job Description
An experienced and compassionate Registered Nurse is needed to provide skilled home health care, supporting patients across their continuum of care in their own homes. This contract position offers an opportunity to exercise autonomy, clinical expertise, and patient advocacyall while facilitating positive health outcomes in the community.
Schedule & Details:
- Full-time, 13-week assignment
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Field-based in patient homes; daily travel required with mileage reimbursement
Responsibilities:
- Complete OASIS assessments and develop individualized patient care plans
- Deliver skilled nursing care, including wound care, medication administration, and chronic disease management
- Evaluate patient progress, promptly communicate changes to the interdisciplinary team and physicians
- Educate patients and families on disease management, medications, and preventative strategies
- Maintain timely documentation in Home Care Home Base (HCHB) system
- Collaborate with therapy, social work, and home health aide colleagues for coordinated care
- Participate in case conferences and staff meetings
- Ensure compliance with regulatory and agency standards
Qualifications:
- Active RN license (state of practice)
- Minimum one year of recent home health RN experience (field experience strongly preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments required
- Demonstrated strong clinical assessment, organizational, and communication skills
- Reliable transportation, current drivers license, and auto insurance
- Ability to work independently and manage a dynamic schedule
- BSN preferred and previous management of a 2532 point weekly caseload is advantageous
- Familiarity with Medicare and home health compliance is a plus
