An opportunity is available for a skilled Registered Nurse (RN) specializing in home health to provide essential nursing care to patients within their homes. This role requires a dedicated professional capable of delivering patient-centered care independently while maintaining high clinical standards and effective communication.

Schedule/Hours:

• Full-time, Monday Friday
• 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop personalized care plans.
• Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, detect changes in condition, and communicate updates with the care team and physicians.
• Educate patients and families regarding disease management, medications, and preventive health practices.
• Maintain accurate and timely documentation using the Home Care Home Base (HCHB) electronic system.
• Collaborate effectively with therapists, social workers, and home health aides to ensure cohesive patient care.
• Participate in case conferences, staff meetings, and ongoing education as required.
• Comply strictly with all regulatory and agency standards in patient care and documentation.

Qualifications & Desired Experience:

• Active RN license in good standing for the state of practice.
• Minimum of 1 year recent experience in home health nursing, preferably with field experience.
• Proficiency in Home Care Home Base (HCHB) electronic documentation required.
• OASIS assessment experience mandatory.
• Strong clinical assessment and critical thinking abilities.
• Reliable transportation with a valid drivers license and auto insurance.
• Ability to work independently managing a flexible schedule in the field.
• Excellent communication, organizational, and time management skills.
• BSN preferred.
• Familiarity with Medicare and home health compliance is a plus.
• Experience managing 2532 points per week desirable.
