An opportunity is available for a skilled Registered Nurse (RN) specializing in home health to provide essential nursing care to patients within their homes. This role requires a dedicated professional capable of delivering patient-centered care independently while maintaining high clinical standards and effective communication.

Schedule/Hours:
- Full-time, Monday Friday
- 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop personalized care plans.
- Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, detect changes in condition, and communicate updates with the care team and physicians.
- Educate patients and families regarding disease management, medications, and preventive health practices.
- Maintain accurate and timely documentation using the Home Care Home Base (HCHB) electronic system.
- Collaborate effectively with therapists, social workers, and home health aides to ensure cohesive patient care.
- Participate in case conferences, staff meetings, and ongoing education as required.
- Comply strictly with all regulatory and agency standards in patient care and documentation.
Qualifications & Desired Experience:
- Active RN license in good standing for the state of practice.
- Minimum of 1 year recent experience in home health nursing, preferably with field experience.
- Proficiency in Home Care Home Base (HCHB) electronic documentation required.
- OASIS assessment experience mandatory.
- Strong clinical assessment and critical thinking abilities.
- Reliable transportation with a valid drivers license and auto insurance.
- Ability to work independently managing a flexible schedule in the field.
- Excellent communication, organizational, and time management skills.
- BSN preferred.
- Familiarity with Medicare and home health compliance is a plus.
- Experience managing 2532 points per week desirable.
