An experienced and compassionate Registered Nurse is needed to provide patient-centered care in a home health setting on a full-time, 13-week contract assignment. You will deliver skilled nursing care directly in patients homes, ensuring both quality and continuity. The ideal candidate is organized, self-motivated, and thrives working independently while collaborating effectively with interdisciplinary teams.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop personalized care plans.
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify any changes in condition, and rapidly communicate with the care team and physicians.
• Educate patients and families on disease processes, treatment plans, medications, and preventive measures.
• Ensure accurate, timely documentation in Home Care Home Base (HCHB).
• Collaborate with physical and occupational therapists, medical social workers, and home health aides for coordinated care delivery.
• Attend case conferences, staff meetings, and relevant continuing education.
• Remain compliant with all regulatory and agency standards.

Requirements:

• Active RN license in good standing within the state of practice
• Minimum 1 year of recent field-based Home Health RN experience
• Proficiency with HCHB and OASIS is required
• Reliable transportation, valid drivers license, and auto insurance
• Strong assessment, organizational, and time management skills
• Ability to independently manage a field-based schedule

Preferred:

• BSN
• Previous caseload experience managing 2532 points per week
• Familiarity with Medicare and home health compliance
