Experienced and compassionate Registered Nurses are needed for a rewarding contract role delivering skilled care directly in patient homes. This role is focused on patient-centered outcomes, requiring you to provide high-quality nursing assessments and individualized care as part of an interdisciplinary team utilizing electronic documentation. Self-motivation, clinical acumen, and strong organizational skills are vital as you travel daily within the assigned territory, building relationships with patients and supporting recovery at home.
Schedule:
- Full-time, 13-week assignment
- Monday Friday, 8:00 AM 5:00 PM
- Anticipated caseload: 2532 points weekly (approximately 35 patient visits per day)
- Field-based; daily travel to patient homes (mileage reimbursement provided)
Key Responsibilities:
- Complete OASIS assessments, develop care plans, and document care using Home Care Home Base (HCHB)
- Deliver skilled nursing interventions: wound care, medication administration, IV therapy, and chronic disease management
- Track patient progress, identify change in status, and communicate with physicians and care teams
- Educate patients and families on clinical conditions, medications, and treatments
- Collaborate with therapists, medical social workers, and home health aides for coordinated care
- Attend case conferences and participate in ongoing training as required
- Maintain strict adherence to regulatory and documentation standards
Qualifications:
- Active RN license in the state of practice (in good standing)
- Minimum 1 year of recent Home Health field experience
- Proficient in Home Care Home Base (HCHB)
- OASIS experience mandatory
- Excellent assessment, communication, and time management skills
- Valid drivers license, reliable transportation, and auto insurance
- Ability to work independently and manage a flexible schedule
- BSN and prior management of 2532 points weekly preferred
