As a Home Health Registered Nurse, you'll perform comprehensive assessments, manage individualized care plans, and contribute to positive patient outcomes. This contract offers a rewarding field-based role for compassionate RNs who excel in independence, clinical judgment, and patient communication.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits daily)

Key Responsibilities:

• Conduct OASIS assessments and develop individualized care plans
• Deliver skilled nursing services, such as wound care, IV therapy, chronic disease management, and medication administration
• Monitor patient conditions, communicate changes promptly, and update care teams as needed
• Educate patients and families on care plans, disease processes, and preventive practices
• Document all visits and interventions in Home Care Home Base (HCHB)
• Coordinate care with interdisciplinary team members, including therapists and social workers
• Participate in case conferences and ensure regulatory compliance

Qualifications:

• Active RN license in good standing
• Minimum 1 year of recent home health experience (field-based experience preferred)
• Proficiency with Home Care Home Base (HCHB)
• Experience using OASIS assessment tools
• Strong assessment, clinical, and organizational abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to work independently while managing a flexible schedule
• Excellent communication skills

Preferred:

• BSN degree
• Experience managing a home health caseload of 2532 points per week
• Familiarity with Medicare and home health compliance

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
