Job Description

Deliver compassionate, high-quality patient care as a Home Health Registered Nurse (RN) providing skilled nursing services in a field-based setting. With this 13-week contract, youll work independently to enhance patient outcomes, support families, and ensure dignity and comfort for those receiving care at home.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, contract assignment

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop personalized care plans for patients in their homes.
  • Deliver skilled nursing interventions such as wound care, medication management, IV therapy, and chronic disease support.
  • Monitor patient progress, communicate changes in condition promptly, and collaborate with the care team and physicians.
  • Educate patients and their families on health conditions, medication regimens, and preventive care.
  • Maintain thorough and timely documentation in Home Care Home Base (HCHB) according to regulatory guidelines.
  • Work closely with a multidisciplinary team, including therapists and social workers, to ensure cohesive patient care.
  • Attend case conferences, participate in staff meetings, and pursue continuing education as needed.
  • Travel daily within the assigned territory; mileage reimbursement is provided.

Qualifications:

  • Active RN license in the state of practice (in good standing)
  • At least 1 year of recent home health (field) experience
  • Proficiency with Home Care Home Base (HCHB) and OASIS required
  • Exceptional clinical assessment, communication, and time management skills
  • Reliable transportation, drivers license, and auto insurance
  • Ability to work independently and efficiently manage a home health caseload

Preferred:

  • BSN degree
  • Experience with caseloads of 2532 points per week
  • Familiarity with Medicare and home health compliance standards

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
