Job Description
An exciting opportunity is available for an experienced and compassionate Home Health Registered Nurse to deliver skilled care to patients in their residences. This field-based, 13-week contract allows you to make a significant impact by supporting patients health, safety, and independencewhile enjoying autonomy and variety in your daily routine.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time contract (13 weeks)
- Typical caseload: 2532 points per week (about 35 patient visits daily)
Key Responsibilities:
- Complete OASIS-driven assessments and develop individualized care plans
- Deliver skilled nursing services: wound care, medication administration, IV therapy, and chronic disease management
- Monitor and evaluate patients progress, updating care plans and communicating with interdisciplinary teams as needed
- Educate patients and families on disease management, medications, and care expectations
- Accurately and promptly document all care activities using Home Care Home Base (HCHB)
- Coordinate services with physical therapists, occupational therapists, social workers, and aides
- Attend required case conferences and team meetings, ensuring compliance with clinical and regulatory standards
Qualifications:
- Active RN license (state of practice), in good standing
- Minimum 1 year of recent home health RN experience (field-based preferred)
- Proficient with Home Care Home Base (HCHB) and OASIS documentation
- Strong assessment, clinical, and critical thinking skills
- Reliable transportation, current drivers license, and auto insurance
- Ability to work independently and coordinate a flexible schedule
- Excellent communication, organization, and time management skills
Preferred:
- Bachelor of Science in Nursing (BSN)
- Prior experience managing 2532 points per week
- Knowledge of Medicare and home health compliance
