Deliver essential skilled nursing care in a field-based role where youll make a meaningful impact on patients lives every day. This full-time, contract opportunity is designed for a Home Health Registered Nurse with strong clinical skills and a passion for supporting individuals in their own homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract duration
• Expected caseload of 2532 points per week (typically 35 patient visits daily)
• Daily travel to patient homes within an assigned territorymileage reimbursement provided

Key Responsibilities:

• Complete thorough OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management
• Evaluate patient progress, identify and report changes in condition, and coordinate with physicians and the interdisciplinary team
• Educate patients and their families on care plans, medications, disease processes, and preventative measures
• Document visits accurately and in real time using Home Care Home Base (HCHB)
• Collaborate closely with therapy, social work, and aide staff for cohesive care
• Attend case conferences and contribute to staff meetings

Qualifications:

• Active, unencumbered RN license
• At least one year of recent field Home Health RN experience
• Required proficiency with OASIS assessments and Home Care Home Base (HCHB) software
• Valid drivers license, auto insurance, and reliable transportation
• Strong independent decision-making, clinical assessment, and organizational skills
• Effective written and verbal communication

Preferred:

• Bachelor of Science in Nursing (BSN)
• Experience managing a 2532 point weekly caseload
• Knowledge of Medicare and home health compliance

$1,845 - $1,952/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
