An opportunity is available for an experienced and compassionate Registered Nurse with strong home health expertise to make a difference in patients lives. This contract role ensures delivery of skilled nursing care in a field-based setting, supporting individuals in the comfort of their homes.

Schedule and Hours:

• Full-time, 13-week contract
• Monday to Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (approximately 35 patient visits daily)
• Daily travel required within assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Complete comprehensive OASIS assessments to develop personalized care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient status, communicate changes promptly to physicians and the care team, and adjust plans as needed.
• Educate patients and families about treatments, medications, disease processes, and preventive care.
• Document visits and care accurately using Home Care Home Base (HCHB).
• Coordinate with a multidisciplinary teamphysical therapists, occupational therapists, social workers, home health aidesto ensure seamless, quality care.
• Participate in ongoing education, case conferences, and staff meetings.
• Follow all regulatory and agency protocols for care delivery and documentation.

Required Qualifications:

• Active RN license (in good standing) for the state of practice
• Minimum 1 year of recent home health field experience
• Proficiency with Home Care Home Base (HCHB)
• OASIS assessment experience
• Strong clinical assessment, communication, and critical thinking skills
• Reliable transportation, drivers license, and auto insurance
• Ability to work independently and manage responsibilities across a flexible geographic territory

Preferred:

• BSN degree
• Experience managing a caseload of 2532 points weekly
• Familiarity with Medicare and home health compliance

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
