A dynamic opportunity is available for an experienced and compassionate Registered Nurse dedicated to home health. This contract role centers on providing skilled, patient-focused care to individuals within their own residences, helping them achieve and maintain optimal health outcomes. The focus is on excellence in clinical skills, assessment, and communication, while working independently in the field and collaborating effectively as part of an interdisciplinary care team.

Schedule:

• Full-time, 13-week contract assignment
• Monday Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes within an assigned territory (mileage reimbursement provided)
• Caseload expectation: 2532 points per week (approximately 35 patient visits daily)

Key Responsibilities:

• Perform OASIS-based comprehensive patient assessments and develop customized care plans
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress and promptly communicate any changes to physicians and the care team
• Provide thorough patient and caregiver education on disease management and medications
• Accurately document all care and communications in Home Care Home Base (HCHB)
• Collaborate closely with other clinicians and participate in case conferences and continuing education
• Uphold all regulatory and documentation standards

Qualifications:

• Current and active RN license in the state of practice
• At least 1 year of recent, hands-on home health nursing experience
• Experience with Home Care Home Base (HCHB) and OASIS is required
• Strong clinical assessment, critical thinking, and time management abilities
• Reliable transportation, valid drivers license, and auto insurance
• BSN and Medicare/home health compliance experience preferred

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
