Key Qualifications:
- Current, unrestricted RN license
- Proven experience in Home Health nursing
- Strong assessment, care planning, and clinical documentation skills
- Proficiency in wound care, medication administration, and chronic disease management
- Ability to work independently while maintaining communication with interdisciplinary teams
- Excellent communication, interpersonal, and organizational abilities
- Valid drivers license and reliable transportation
Primary Responsibilities:
- Conduct comprehensive, in-home assessments of patients health status and living environments
- Develop, implement, and evaluate individualized care plans
- Administer medications, treatments, and injections as prescribed
- Monitor and document patient progress, updating care plans as needed
- Educate patients and families on disease process, medications, and therapy plans
- Collaborate with physicians, therapists, and other providers to ensure coordinated, high-quality care
- Maintain accurate clinical records in compliance with regulatory standards and agency policies
- Advocate for patient needs, promoting safety and comfort in a home environment
