Embark on a meaningful contract role as a Home Health Registered Nurse, delivering high-quality, compassionate care to patients in their own homes. This position is ideal for self-driven professionals who thrive independently, value patient-centered care, and excel in managing complex care requirements in diverse home environments.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Field-based role with daily travel to patient homes (mileage reimbursement provided)
• Caseload: 2532 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Complete thorough OASIS assessments and develop tailored care plans.
• Provide skilled clinical interventions such as wound management, medication administration, IV therapies, and chronic disease support.
• Monitor patient progress, swiftly identifying and addressing changes in condition; communicate with physicians and care teams as appropriate.
• Deliver patient and family education on health management topics, medication regimens, and preventive care strategies.
• Document all assessments, care visits, and communications using Home Care Home Base (HCHB) electronic system.
• Coordinate with a multidisciplinary team including therapists, social workers, and aides to deliver integrated care.
• Attend case conferences, participate in staff development, and maintain adherence to regulatory standards.

Qualifications:

• Active RN license in good standing
• At least 1 year of recent field-based home health experience
• Proficiency with HCHB and OASIS assessment tools
• Strong clinical judgment and excellent communication skills
• Valid drivers license, reliable transportation, and current auto insurance
• BSN and previous experience with a similar caseload, plus familiarity with Medicare/home health compliance standards, preferred

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
