Deliver high-quality, patient-centered care as a Home Health Registered Nurse providing skilled nursing services directly in patients homes. Leverage your clinical expertise and compassion to support patients in their recovery and ongoing health management throughout a 13-week contract assignment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, with daily travel to patient homes within your assigned area
• Caseload of 2532 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Complete comprehensive OASIS assessments to shape individualized care plans.
• Deliver skilled nursing interventions, including wound care, IV therapy, chronic disease management, and medication administration.
• Closely monitor each patients progress, promptly recognizing and reporting changes in condition.
• Educate patients and families on diagnoses, medication regimens, preventive measures, and self-care techniques.
• Maintain thorough and timely documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated, holistic care.
• Attend case conferences, contribute in staff meetings, and participate in ongoing professional development.
• Uphold all regulatory standards for documentation and care delivery.

Required Qualifications:

• Active RN license in good standing
• At least one year of recent home health nursing experience (field-based strongly preferred)
• Proficient with Home Care Home Base (HCHB) and OASIS
• Excellent assessment, communication, and organization skills
• Comfortable working independently with strong time management
• Reliable vehicle, valid drivers license, and auto insurance

Preferred Qualifications:

• BSN degree
• Familiarity managing a caseload of 2532 points weekly
• Knowledge of Medicare and home health compliance standards
