Job Description

An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care to patients within their homes, focusing on quality, continuity, and patient-centered services. This full-time, 13-week contract position involves managing a caseload of 2532 points weekly, typically seeing 35 patients per day across a defined territory. The role demands self-motivation, strong organizational skills, and the ability to work independently while maintaining clinical excellence and clear patient communication.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Provide skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, detect changes, and communicate effectively with care teams and physicians.
  • Educate patients and families on disease management, treatments, medications, and preventive care.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate care.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Comply with regulatory and agency standards for documentation and patient care.

Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum of one year recent home health experience; field experience preferred.
  • Proficiency with HCHB and OASIS required.
  • Strong clinical assessment and critical thinking abilities.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to manage a flexible, field-based schedule independently.
  • Excellent communication, time management, and organizational skills.

Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Experience managing 2532 points per week.
  • Knowledge of Medicare and home health compliance standards.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM.
