A skilled and dedicated Home Health Registered Nurse is needed for a full-time, 13-week contract. This field-based position focuses on delivering high-quality nursing care to patients in their homes, ensuring a strong patient-centered approach, clear clinical documentation, and close collaboration with interdisciplinary teams.
Schedule & Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload expectation: 2532 points per week (typically 35 patient visits daily)
- Home-based care within an assigned territory (daily travel; mileage reimbursement included)
- Documentation via Home Care Home Base (HCHB); OASIS assessments required
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Administer skilled nursing services, including wound care, medication setup, IV therapy, and disease management
- Monitor patient progress and communicate proactively with care teams and physicians
- Provide patient and family education regarding diagnoses, treatments, medications, and preventive care
- Maintain timely, accurate electronic documentation
- Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides
- Participate in team meetings, case conferences, and required continuing education
- Ensure compliance with all relevant regulatory and agency standards
Qualifications:
- Active Registered Nurse license in good standing
- Minimum one year of recent Home Health experience (field work preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Reliable transportation, valid drivers license, and auto insurance
- Excellent assessment, organizational, and communication skills
- BSN and Medicare/home health compliance knowledge preferred
- Experience managing 2532 points per week is a plus
