Deliver personalized care and make a difference as a Home Health Registered Nurse (RN). This rewarding opportunity allows you to provide skilled nursing services to patients in their homes, ensuring they receive dedicated attention and expert support. The role is ideally suited for clinicians who thrive in field-based settings, work well independently, and value building relationships with patients and their families across the continuum of care.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- 13-week contract assignment
- Typical caseload: 2532 points per week (averaging 35 patient visits per day)
Core Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized patient care plans
- Deliver skilled nursing interventions, including wound care, IV therapy, and chronic disease management
- Monitor patient conditions, adapting plans and coordinating with care teams and physicians as needed
- Educate patients and families on care protocols, medications, and disease management strategies
- Maintain accurate, timely documentation in Home Care Home Base (HCHB)
- Collaborate with interdisciplinary colleaguesPT, OT, social workers, and home health aidesto ensure holistic care delivery
- Participate in case conferences, staff meetings, and ongoing education
Required Qualifications:
- Active, unencumbered RN license in state of practice
- Minimum of one year recent home health nursing experience
- Proficiency with OASIS and Home Care Home Base (HCHB) documentation systems
- Reliable transportation, valid drivers license, and auto insurance
- Strong clinical, communication, and organizational skills
Preferred:
- BSN degree
- Previous experience handling a similar home health caseload
- Working knowledge of Medicare and compliance requirements
