Delivering high-quality, patient-centered care in the comfort of home settings, this role calls for a skilled Home Health Registered Nurse to work closely with patients and families to optimize health outcomes. Ideal candidates are organized, independent, and committed to clinical excellence, with a proven ability to provide comprehensive care and exceptional support across a diverse patient population.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor clinical progress, respond to changes in condition, and maintain open communication with interdisciplinary teams and physicians.
• Educate patients and caregivers on disease management, medications, and treatment plans.
• Maintain complete, timely documentation using Home Care Home Base (HCHB).
• Coordinate with physical therapists, occupational therapists, home health aides, and social workers for seamless patient care.
• Participate in staff meetings, case conferences, and ongoing training as needed.
• Adhere to agency policies, regulatory guidelines, and documentation requirements.

Qualifications:

• Active, unencumbered RN license in the state of practice.
• Minimum of one year of recent home health experience, preferably in the field.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical, assessment, and critical thinking skills.
• Excellent communication, time management, and organizational abilities.
• Reliable transportation, valid drivers license, and current auto insurance.
• BSN and Medicare/home health compliance experience preferred.
• History of managing 2532 points per week desirable.
