Bring your clinical expertise and passion for patient-centered care to a dynamic Home Health Registered Nurse role focused on supporting individuals in the comfort of their own homes. As a Home Health RN, you will play a vital part in managing patient care, building relationships, and contributing to positive health outcomes through compassionate, skilled nursing.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Field-based with daily travel within an assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress, assess for changing conditions, and communicate promptly with the care team and physicians
- Provide education on disease processes, medication administration, and preventive care to patients and families
- Accurately document all care activities using Home Care Home Base (HCHB)
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides to deliver coordinated care
- Participate in staff meetings, case conferences, and ongoing professional development
Qualifications:
- Active RN license in good standing
- At least 1 year of recent Home Health RN experience (field-based preferred)
- Proficiency with OASIS and Home Care Home Base (HCHB)
- Strong assessment, communication, and time management skills
- Reliable transportation, valid drivers license, and current auto insurance
- BSN and Medicare compliance knowledge desirable
