Dedicated registered nurses with a passion for delivering patient-centered care in the home setting will find this contract opportunity both rewarding and vital. In this role, you will ensure patients receive high-quality, skilled nursing care in the comfort of their homes, fostering recovery and independence while collaborating closely with interdisciplinary teams.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract duration
- Manage a caseload of 2532 points per week (typically 35 patient visits per day)
- Daily travel within an assigned territory, with mileage reimbursement provided
Qualifications:
- Valid, active RN license in state of practice (in good standing)
- Minimum of 1 year of recent home health (field) nursing experience required
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- Solid experience completing OASIS assessments
- Strong clinical assessment, independent decision-making, and organizational abilities
- Reliable transportation, current drivers license, and proper auto insurance
- Excellent communication and patient education skills
Preferred Experience:
- BSN degree
- Previous experience managing 2532 points per week
- Familiarity with Medicare regulations and home health compliance
Key Responsibilities:
- Perform comprehensive OASIS assessments and design personalized care plans
- Deliver skilled nursing services: wound care, medication administration, IV therapy, and chronic disease management
- Educate patients and families about health conditions, treatment plans, and preventive care
- Maintain accurate and timely documentation using HCHB
- Collaborate with therapists, social workers, and home health aides to ensure coordinated care
- Attend case conferences and contribute to continuous quality improvement
