Job Description

Provide skilled, patient-focused care as a Home Health Registered Nurse (RN) delivering nursing services directly in patients homes. This role calls for a self-motivated, organized professional comfortable working independently while maintaining high clinical standards and compassionate communication. You will perform thorough assessments, develop care plans, educate patients and families, and collaborate with multidisciplinary teams to support positive outcomes.

Key Responsibilities:

  • Conduct comprehensive assessments using OASIS and develop individualized care plans for home health patients.
  • Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify changes in condition, and communicate promptly with physicians and care teams.
  • Provide patient and family education on disease management, treatment adherence, and preventive care.
  • Maintain accurate, timely documentation in Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and home health aides to coordinate care delivery.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Comply with all regulatory agency standards and documentation requirements.

Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum 1 year recent home health nursing experience (field-based preferred).
  • Proficiency with Home Care Home Base (HCHB) and OASIS essential.
  • Strong clinical assessment, critical thinking, and independent work skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Effective communication, time management, and organizational abilities.
  • BSN preferred.
  • Experience managing a caseload of 2532 points per week.
  • Knowledge of Medicare and home health compliance standards preferred.

Schedule and Location:

  • Full-time 13-week contract.
  • Monday through Friday, 8:00 AM 5:00 PM.
  • Caseload typically ranges from 25 to 32 points (35 patient visits daily).
  • Field-based role requiring daily travel to patient homes within assigned territory; mileage reimbursement provided.
