Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care to patients in their homes. This role involves delivering quality, patient-centered care with a focus on comprehensive assessments, individualized care planning, and coordination with interdisciplinary healthcare teams. Ideal candidates are self-motivated, organized, and able to work independently while maintaining clinical excellence and clear patient communication.

Location:
This position is based in Elizabeth, NJ, offering the opportunity to serve patients within the local community in their home environments.

Contract Details:

  • Full-time, 13-week contract
  • Schedule: Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week, typically 35 patient visits per day
  • Field-based care with daily travel to patient homes (mileage reimbursed)

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans
  • Provide skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
  • Monitor patient progress, identify changes, and communicate updates to the care team and physicians
  • Educate patients and families on treatment plans, medications, and preventive care
  • Maintain accurate documentation using Home Care Home Base (HCHB)
  • Collaborate with therapists, social workers, and aides for coordinated care
  • Participate in case conferences, staff meetings, and continuing education
  • Adhere to all regulatory and agency standards for documentation and patient care

Qualifications:

  • Active RN license in good standing for the state of practice
  • Minimum of 1 year recent home health experience (field experience preferred)
  • Proficiency in Home Care Home Base (HCHB) and OASIS
  • Reliable transportation, valid drivers license, and auto insurance
  • Strong clinical assessment, critical thinking, communication, and organizational skills
  • Ability to work independently with a flexible field-based schedule

Preferred:

  • BSN degree
  • Previous management of 2532 points weekly
  • Knowledge of Medicare and home health compliance standards
