Job Description
An experienced and compassionate Registered Nurse with home health expertise is needed to provide skilled nursing care directly in patients homes within the Elgin, IL area. This role requires a highly motivated professional who thrives working independently while maintaining clinical excellence and delivering patient-centered care.
As a home health RN, you will conduct comprehensive assessments using OASIS, develop personalized care plans, and provide skilled interventions including wound care, medication administration, and chronic disease management. You will closely monitor patient progress, communicate changes with healthcare team members and physicians, and educate patients and families on treatment plans and preventive care. Documentation will be completed using the Home Care Home Base (HCHB) electronic system, ensuring compliance with regulatory and agency standards. Collaboration with therapists, social workers, and home health aides is essential to coordinate multidisciplinary care effectively.
Key Responsibilities:
- Conduct thorough OASIS assessments and develop individualized care plans
- Provide skilled nursing treatments and monitor patient status
- Educate patients and caregivers about disease management and medications
- Maintain accurate, timely documentation in HCHB
- Communicate changes in condition with care teams and physicians
- Participate in case conferences and continuing education
- Adhere to all regulatory standards and agency policies
Qualifications and Desired Experience:
- Active RN license in good standing
- Minimum 1 year recent home health experience with fieldwork preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS mandatory
- Strong clinical assessment and critical thinking abilities
- Reliable transportation, valid drivers license, and auto insurance
- Excellent communication, organizational, and time-management skills
- BSN preferred
- Familiarity with Medicare and home health compliance standards is a plus
Position Details:
- Contract Type: Full-time, 13-week contract
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points weekly (35 patient visits/day)
