An opportunity is available for an experienced and compassionate Registered Nurse to provide skilled nursing care in a home health setting. The role emphasizes quality, patient-centered care and independent clinical judgment as you support individuals in the comfort of their own homes.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

What Youll Do:

• Conduct OASIS assessments and develop individualized care plans for patients in their homes.
• Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly addressing changes in condition and communicating with the healthcare team and physicians.
• Educate patients and their families regarding disease processes, treatment protocols, medications, and preventive care.
• Utilize the Home Care Home Base (HCHB) electronic documentation system for accurate and timely records.
• Coordinate care with an interdisciplinary team including therapists, social workers, and aides.
• Attend case conferences and staff meetings, and maintain compliance with regulatory standards.

Qualifications:

• Active RN license in good standing.
• Minimum of 1 year of recent field experience in Home Health.
• Required proficiency with OASIS and Home Care Home Base (HCHB).
• Strong clinical assessment, critical thinking, and organizational skills.
• Self-directed with the ability to work independently and manage schedules.
• Reliable transportation, valid drivers license, and auto insurance.

Preferred:

• BSN degree.
• Previous experience managing a caseload of 2532 points per week.
• Familiarity with Medicare and home health compliance.

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
