Deliver skilled and compassionate care to patients where they feel most comfortabletheir homesas a Home Health Registered Nurse. This field-based, full-time contract offers the chance to make a significant impact on patient outcomes through clinical excellence, effective case management, and education.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments to develop and manage individualized care plans.
• Provide a range of skilled nursing services (wound care, IV therapy, medication administration, and chronic disease management).
• Closely monitor patient progress, identify health status changes, and maintain open communication with interdisciplinary teams and physicians.
• Educate patients and their families on disease management, medications, and preventive strategies.
• Document all patient care in Home Care Home Base (HCHB) accurately and in a timely manner.
• Coordinate care with physical therapists, occupational therapists, medical social workers, and home health aides.
• Attend case conferences, staff meetings, and participate in continuing education as needed.
• Ensure compliance with regulatory and agency requirements.

Qualifications:

• Active, unencumbered RN license in the state of practice.
• Minimum 1 year of recent Home Health nursing experience (field experience strongly preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Demonstrated strong clinical assessment and independent critical thinking skills.
• Efficient time management with the ability to handle a caseload of 2532 points per week.
• Valid driver's license, reliable transportation, and active auto insurance.
• Excellent communication and organizational skills.
• BSN and knowledge of Medicare/home health compliance preferred.
