A rewarding opportunity is available for a skilled and compassionate Registered Nurse specializing in hospice and home health care. In this position, you will provide essential end-of-life support and care to patients in their homes, assisted living residences, or nursing facilities. This role centers on ensuring dignity, comfort, and quality of life for patients while supporting families throughout the hospice journey.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM (occasional weekend or on-call may be required)
- Caseload of approximately 45 patient visits per day
Qualifications:
- Active RN license in good standing
- At least 1 year of recent hospice nursing experience
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- Strong clinical assessment and communication skills
- Sound knowledge of pain management and end-of-life care principles
- Reliable transportation, valid drivers license, and auto insurance
- Ability to manage a field-based schedule independently
- Compassionate, emotionally resilient, and supportive demeanor
- BSN and prior palliative care experience preferred
- Familiarity with Medicare hospice regulations is a plus
Key Responsibilities:
- Complete in-depth patient nursing assessments and develop customized care plans
- Deliver skilled nursing services with a focus on symptom management and patient comfort
- Administer medications and other physician-ordered treatments
- Monitor changes in patient conditions and communicate promptly with the hospice care team
- Educate patients, families, and caregivers on disease progression, comfort measures, and emotional support
- Work collaboratively with interdisciplinary team members to address overall patient and family needs
- Maintain accurate and timely documentation using HCHB
- Participate in team meetings and continuous quality improvement initiatives
