Job Description
Deliver compassionate, skilled care as a Hospice Registered Nurse (RN) supporting patients and families in their homes and community residential settings. This 13-week contract provides the opportunity to make a meaningful impact during the most critical phase of a patients life journey. The role focuses on expertly coordinating and delivering end-of-life care with a sensitive, patient-centered approach.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend/on-call rotation may be required
Key Responsibilities:
- Conduct comprehensive nursing assessments and develop individualized hospice care plans
- Provide direct skilled nursing care, emphasizing symptom management, pain control, and comfort measures
- Administer medications and treatments according to physician orders and hospice protocols
- Monitor and report patient condition changes to the interdisciplinary team
- Educate patients, families, and caregivers on disease process, symptom management, and emotional support
- Collaborate with social workers, chaplains, aides, and volunteers to address holistic needs
- Accurately document all care and communication using Home Care Home Base (HCHB)
- Participate in team meetings, case conferences, and quality improvement initiatives
- Ensure compliance with all hospice policies and regulatory standards
Requirements:
- Active, unencumbered RN license in state of practice
- Minimum 1 year of recent hospice RN experience required
- Proficiency with Home Care Home Base (HCHB) is essential
- Strong assessment, communication, clinical, and organizational skills
- Ability to work independently, with reliable transportation, valid drivers license, and auto insurance
- Demonstrated compassion and emotional resilience
Preferred Attributes:
- BSN degree
- Palliative or end-of-life care experience
- Familiarity with Medicare hospice documentation standards
