Job Description
Seeking a dedicated and experienced Registered Nurse with a passion for home health to provide high-quality care to patients in a field-based setting. This full-time, 13-week contract position involves delivering patient-centered care in their homes, prioritizing both clinical excellence and compassionate support.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive patient assessments using the OASIS tool and develop individualized care plans.
- Provide skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
- Conduct 35 patient visits per day (caseload: 2532 points per week).
- Use Home Care Home Base (HCHB) for timely and accurate clinical documentation.
- Monitor patient progress, identify changes in condition, and communicate vital information to physicians and the interdisciplinary team.
- Educate patients and families regarding disease processes, medications, treatment plans, and preventive measures.
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides to ensure seamless care coordination.
- Participate in case conferences, staff meetings, and ongoing professional development.
- Maintain compliance with all regulatory, clinical, and documentation standards.
- Travel daily to provide care within the assigned territory (mileage reimbursement provided).
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum 1 year recent field-based Home Health RN experience.
- Proficiency in Home Care Home Base (HCHB) and OASIS required.
- Strong clinical assessment, critical thinking, and organizational skills.
- Reliable transportation, valid drivers license, and current auto insurance.
- Ability to work independently while managing priorities.
- Excellent communication and time management abilities.
- BSN preferred and experience managing 2532 points per week is advantageous.
- Familiarity with Medicare and home health compliance is beneficial.
