Job Description
Provide skilled nursing care to patients in their homes as a Home Health Registered Nurse (RN) dedicated to delivering quality, compassionate, and patient-centered care. This full-time, 13-week contract position involves managing a caseload of 2532 points per week, typically 35 patient visits daily, within an assigned territory.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, identify changes, and communicate promptly with care teams and physicians
- Educate patients and families on disease management, treatment plans, and preventive care
- Accurately document care and assessments using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams including therapists, social workers, and home health aides
- Participate in case conferences, staff meetings, and maintain adherence to agency and regulatory standards
Qualifications:
- Active RN license in good standing in the state of practice
- Minimum of 1 year recent home health experience; field experience preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage flexible field-based scheduling
- Excellent communication, time management, and organizational skills
Preferred:
- Bachelor of Science in Nursing (BSN)
- Experience managing 2532 points weekly
- Familiarity with Medicare and home health compliance standards
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
